Living with a long-term health condition means making decisions about food every single day — often with conflicting advice from every direction. Our Accredited Practising Dietitians work alongside your GP and treating team to turn your diagnosis, your blood results and your real life into a plan you can actually follow.

  • ✓ Accredited Practising Dietitians (APDs), registered with Dietitians Australia
  • ✓ Six Melbourne clinics, home visits, and telehealth Australia-wide
  • ✓ GP referrals welcome — Medicare, NDIS, DVA and private health rebates
  • ✓ A written action plan and a clear pathway from your first session

Conditions We Support

Chronic conditions rarely arrive one at a time. Kidney disease often follows diabetes; high blood pressure and cholesterol travel together; fatty liver and insulin resistance are closely linked. Our dietitians are trained to manage these overlaps rather than treat each one in isolation.

Digestive Conditions

Coeliac disease, inflammatory bowel disease, IBS and food intolerances are supported through our Gut Health & IBS service, where five of our dietitians hold the Monash University Low FODMAP Diet credential.

A note on scope. Dietitians provide nutrition care as part of your wider treatment team. We work alongside your GP, and your nephrologist, cardiologist, endocrinologist or oncologist where relevant — we do not replace medical treatment, diagnose conditions, or advise on medications.

Your Action Plan and Your Pathway

Nobody leaves a NutriBlooms consultation with a photocopied handout and a vague instruction to eat better. Every client receives two things in writing.

Your action plan

A personalised plan built around your diagnosis, your pathology results, your medications, your culture and the way you actually eat. It sets out practical objectives, the goals you and your dietitian agree on together, and the milestones you’ll use to track whether the plan is working. These are collaborative goals — not fixed numeric targets or promised outcomes.

Your pathway

A clear map of what happens next: how many sessions we anticipate, what each one will cover, when we’ll review your bloods, how we communicate with your GP, and how ongoing support is structured once the early work is done. You’ll know where you’re going before you leave the first appointment.

What Your First Consultation Includes

Your initial session runs for 60 minutes, in clinic or via telehealth.

  • ✓ A full review of your health history, diagnosis, medications and pathology results
  • ✓ A detailed diet history and lifestyle assessment
  • ✓ Body composition analysis on our InBody scanner, measuring muscle mass, body fat and fluid — available at face-to-face appointments
  • ✓ Access to the InBody app so you can follow your own results over time
  • ✓ An individualised meal plan built around your condition and your preferences
  • ✓ Practical food label guidance relevant to your condition
  • ✓ Your written action plan and pathway

Review sessions

Follow-up appointments reassess your body composition, revisit your nutritional needs as your condition and treatment change, update your meal plan, and check progress against the milestones you agreed. Visit our Consultations page for current fees and packages.

Why a Clinic.

Built-in collaboration

Complex cases rarely stay in one lane. When kidney disease overlaps with diabetes and unintentional weight loss, your dietitian doesn’t work it out alone — they case-conference with colleagues in the clinic and with our Principal Dietitian, Racha Slim, to get a second opinion the same week.

A room designed to be calm

Managing a long-term condition is tiring enough. Our clinics are designed to feel calm and comfortable rather than clinical — a lounge-inspired space, quiet music, self check-in, and a cup of tea while you wait. It’s a small thing that makes an honest conversation about food a lot easier.

Structured Programs

Alongside one-to-one consultations, we run two structured programs for chronic conditions.

Cholesterol Lowering Diet Program

A structured, dietitian-led program for people working on their cholesterol and heart health, with coaching and progress tracking throughout.

Life! Program — free, government-funded

A free Victorian Government program to help reduce your risk of type 2 diabetes, heart disease and stroke. Seven sessions over 12 months, funded by the Victorian Government and managed by Diabetes Victoria. Our dietitian Kineta Sulemani is an accredited Life! Program facilitator.

Referrals, Rebates and Fees

Medicare — the rules changed on 1 July 2025

The old GP Management Plan and Team Care Arrangements have been replaced by a single GP Chronic Condition Management Plan (GPCCMP). Three things are easier for you now:

  • ✓ Your GP can refer you with an ordinary referral letter — the old Medicare form is gone
  • ✓ Your GP no longer needs to coordinate with two providers before referring
  • ✓ The referral doesn’t have to name a specific dietitian, so the choice is yours

A GPCCMP gives you up to five subsidised individual allied health services per calendar year (ten for people of Aboriginal or Torres Strait Islander descent), and the referral is valid for 18 months from your first session. If you had a GP Management Plan in place before 1 July 2025, you can keep using it until 30 June 2027. Details are on the Services Australia website.

About our fees. NutriBlooms is not a bulk-billing clinic. Where you have a valid GPCCMP referral, Medicare pays a rebate towards your consultation and a gap fee applies. We’ll tell you the exact out-of-pocket cost before you book — no surprises.

Other funding

  • NDIS — we’re an NDIS-registered provider supporting plan-managed and self-managed participants. NDIS dietitian services
  • DVA — we support veterans referred under DVA arrangements
  • Private health insurance — most extras policies include dietetics, subject to your level of cover
  • No referral needed — you can book privately at any time without seeing your GP first

Frequently Asked Questions

Do I need a GP referral to see a dietitian for a chronic condition?

No. You can book a private appointment at any time without a referral. A referral only matters if you want a Medicare rebate — for that, your GP needs to prepare a GP Chronic Condition Management Plan (GPCCMP) and give you a referral letter. Since 1 July 2025 that letter no longer needs to be on a special Medicare form.

Do you bulk bill?

No — NutriBlooms is not a bulk-billing clinic. With a valid GPCCMP referral, Medicare pays a rebate towards your consultation and a gap fee applies. We’ll confirm your exact out-of-pocket cost before you book.

How many Medicare-subsidised sessions can I get?

A GPCCMP allows up to five individual allied health services per calendar year, or ten for people of Aboriginal or Torres Strait Islander descent. Those five are shared across all allied health providers you see, not five per profession. Your referral stays valid for 18 months from your first session.

I have more than one condition. Do I need separate appointments?

No. Chronic conditions overlap constantly — diabetes with kidney disease, cholesterol with fatty liver — and treating them separately usually produces contradictory advice. Your dietitian manages them together in one plan, and can case-conference with colleagues in the clinic when a case is especially complex.

What stage of kidney disease do you support?

Every stage, from early-stage CKD through to dialysis and transplant. Nutrition needs change substantially between stages — an approach suited to stage 2 can be actively unhelpful at stage 5 — so your plan is built around your current eGFR and blood results, and we work alongside your nephrology team rather than in place of it.

Can I have my appointment by telehealth?

Yes. We offer telehealth consultations Australia-wide, and they attract the same Medicare rebate as face-to-face appointments where you have a valid referral. The one difference is body composition analysis, which needs you in the clinic. We also offer home visits.

Will you talk to my GP?

Yes. Where you’ve been referred, we write back to your referring GP so your care stays coordinated. We can also work alongside your treating team — nephrologist, cardiologist, endocrinologist or oncologist — where that’s relevant to your care.

How many sessions will I need?

It depends on your condition, how complex it is and what you want to work on. Your dietitian will discuss an anticipated number of sessions with you as part of your pathway at the first appointment, so you know what you’re committing to before you go ahead.

Book with an Accredited Practising Dietitian at any of our six Melbourne clinics, by home visit, or by telehealth anywhere in Australia. GP referrals welcome under a GPCCMP — Medicare, NDIS, DVA and private health rebates apply, with a gap fee. No referral needed to book privately.
