Home Care Dietitian in Melbourne for Support at Home & Aged Care

NutriBlooms’ Accredited Practising Dietitians provide home care dietitian services across Melbourne, supporting older adults to eat well, stay strong and live independently at home. We see clients under the Support at Home program — through home visits across Melbourne metro, in residential aged care facilities, at our six clinic locations, or by telehealth Australia-wide.

Families, care partners, support coordinators and GPs are all welcome to refer. We aim to be in touch within 48 hours of receiving your referral.

Dietitian Services Under the Support at Home Program

On 1 November 2025, the Australian Government’s Support at Home program replaced the Home Care Packages Program and Short-Term Restorative Care. If you were receiving a Home Care Package, you were transitioned automatically with an equivalent level of funding — and you can continue to include dietetic services in your care.

Dietitian services sit within the Clinical Supports (Allied Health and Other Therapeutic Services) category of Support at Home. Under current program rules, participants do not contribute to the cost of clinical supports — dietetic care is funded from your Support at Home budget with no co-contribution.

We also welcome referrals under the Restorative Care Pathway — the short-term Support at Home pathway that funds coordinated allied health services to help you maintain or regain function and independence at home.

New to aged care funding? Everything starts with a My Aged Care assessment — visit My Aged Care or call 1800 200 422. Your assessment determines your classification and budget. We’re happy to help you and your family understand where dietetic support fits in your plan.

Menu Reviews for Providers, Facilities and Community Services

Alongside individual care, NutriBlooms offers a dedicated dietitian Menu Review service for organisations — residential aged care facilities, home care and Support at Home providers, NDIS and SIL homes, community meal services and childcare centres.

Every review is delivered as a written report with prioritised, practical recommendations to help you meet food and nutrition expectations under the strengthened Aged Care Quality Standards. Reviews are available on-site across Melbourne metro or remotely Australia-wide, with optional six-month follow-up reviews and staff education available as add-ons.

How It Works

1. Send a referral or call us

GPs, care partners, support coordinators, providers and families can all refer. We aim to be in touch within 48 hours of receiving your referral to arrange a time that suits you.

2. Initial assessment

Your first appointment can take place at home (Melbourne metro), at one of our six clinics, in your residential aged care facility, or by telehealth. We take the time to understand your medical history, medications, current eating patterns and — most importantly — what matters to you.

3. Your personalised action plan

You’ll receive a clear, personalised action plan with practical objectives, agreed goals and milestones, plus a defined pathway that maps your next steps and how your ongoing support is structured — so you, your family and your care team always know what happens next.

4. Ongoing reviews

We review progress with you, adjust the plan as your needs change, and keep your GP and care team in the loop along the way.

Experienced, Team-Based Care

The team behind your care

Simon Salerno, Accredited Practising Dietitian, brings extensive experience in malnutrition, aged care and tube feeding. Kineta Sulemani, Accredited Practising Dietitian, has advanced training in Home Enteral Nutrition and tube feeding.

Complex cases are never handled alone — our dietitians case-conference within the team, including with Founder and Principal Dietitian Racha Slim, whenever a second opinion helps your care.

Accredited and regulated

Every NutriBlooms dietitian is an Accredited Practising Dietitian (APD) — the credential recognised by Medicare, the NDIS and private health insurers. NutriBlooms is a registered NDIS provider, and our care is guided by current evidence and Australian clinical guidelines.

Frequently Asked Questions

Is a dietitian covered under the Support at Home program?

Dietitian services sit within the Clinical Supports (Allied Health and Other Therapeutic Services) category of Support at Home. Under current program rules, participants do not contribute to the cost of clinical supports. Whether dietetics is included in your plan depends on your My Aged Care assessment — your care partner or coordinator can confirm this, or call us and we’ll point you in the right direction.

What happened to Home Care Packages?

On 1 November 2025, the Support at Home program replaced the Home Care Packages Program and Short-Term Restorative Care. If you were receiving a Home Care Package, you were transitioned automatically with an equivalent level of funding, and you can continue to include dietetic services in your care.

Do I pay anything for dietitian visits under Support at Home?

Under current program rules, clinical supports — including dietetics — do not attract a participant contribution and are funded from your Support at Home budget. If you’re self-funded or accessing our services privately, we’ll explain all fees clearly before your first appointment.

Is there a travel fee for home visits?

Yes — home visits include a separate travel fee based on your location. Call us on 0431 189 792 and we’ll confirm the fee for your suburb before anything is booked.

Can you visit me at home?

Yes — our dietitians provide home visits across Melbourne metro. You can also see us at our clinics in Blackburn, Ringwood, Lilydale, Dandenong South, Roxburgh Park and Epping/Lalor, or by telehealth anywhere in Australia.

Do you see residents in aged care facilities?

Yes. We provide individual dietitian consultations in residential aged care facilities, and we also offer a dedicated Menu Review service for facilities and providers, delivered as a written report with prioritised recommendations.

How quickly can we start?

We aim to be in touch within 48 hours of receiving your referral. Appointment timing then depends on your needs, location and preferred appointment type — we’ll work with you to find the earliest suitable time.

What is the Restorative Care Pathway?

The Restorative Care Pathway is a short-term Support at Home pathway that funds coordinated allied health services to help you maintain or regain function and independence. NutriBlooms welcomes dietetic referrals under this pathway.

Can you help with special diets, allergies or cultural food preferences?

Yes. Our dietitians support a wide range of therapeutic and special diets — including texture-modified, vegetarian, gluten-free and allergy-aware eating — and tailor recommendations to your preferences, culture and routines. Italian- and Arabic-speaking dietitians are available.

Do you offer telehealth appointments?

Yes. If a home or clinic visit isn’t practical, we provide telehealth consultations Australia-wide, including follow-up reviews.

Whether you’re an older adult, a family member, a support coordinator or a Support at Home provider, we’d love to help. Send a referral online or call 0431 189 792 — home visits across Melbourne metro, six clinic locations and telehealth Australia-wide.
